Medical and Health Information: We collect medical records and other health-related information necessary for the surrogacy process from a variety of sources including from the questionnaires and written correspondence we receive from you.  The medical records we collect are fulsome and may, among other things, contain information about your pregnancy and delivery history, general health history, substance abuse history, history of sexually transmitted disease and other medical health history.  To assist with records collection or insurance review, we sometimes acquire your username and password for certain portals.

Financial Information: If you make payments for services, we may collect financial details such as credit card information, billing address, and transaction history.  We also collect your username information for certain payment apps so that we can assist with payments and reimbursements between our parents and surrogates during their matching and screening phases.
  4. Legal and Background Information: We obtain background checks for surrogates and intended parents, and may also receive other sensitive information including legal documents as required to perform services.
  5. Website Usage Information: We may collect information about your use of our website, including IP addresses, device information, and cookies, to improve our services and understand user preferences. We may receive information about you if other users of third-party applications give us access to their profiles and you are one of their “connections” or information about you is otherwise accessible through your “connections’” web page, profile page, or similar page on a social networking or other third-party web sites or interactive service.
